[Vv] transformation matrix

Hi,

Thanks for developing this great tool. I am wondering how vv derive the
transformation matrix when loading a dicom image? I calculated it based on
the tags "Image Position Patient" and "Image Orientation Patient", but the
outcome was always different from the one displayed in vv.
