[Vv] clitkDicomRTStruct2Image

Hi everyone,

Thank you for your feedback Neree and Benoit.
The problem came from the previous commit I did to solve your last 
issue.

I don't precisely understand the difference between this dicom RTStruct 
and the others. But I think I solved the problem with this new commit:
https://github.com/open-vv/vv/commit/fa3343f1df86c87e892cafbdda1f6b0b0b1874b0
I undid the modifications of the previous commit and I change the 
position of the point along the z-axis just before to extrude it.

Don't hesitate to ask me if you have new problems
