[Dcmlib] Initial support for MINGW32

Good news everyone ! (*)

GDCM finally support (very poorly for now) MinGW32:

Compilation is fine, no warnings all tests are passings. The only thing 
I have not figured out yet is why the compilation fails a link time when 
doing shared libraries...

I hope to fix that in the next few weeks. But for now you can use GDCM 
throught Mingw32 using static libs !
